About Us

North Uttlesford Primary Care Network (PCN) is a collaboration of the four GP surgeries based in North Uttlesford, working together with the community, mental health, social care, pharmacy, hospital and voluntary services to deliver locally led and locally delivered health services.

Our Core Belief

Our core belief is that high quality and sustainable primary care is pivotal to this ambition.  To succeed we will embrace change and take a disciplined approach to the prioritisation of resources as well as developing high performing partnerships.  

Against an ever-changing landscape of increasing demand and finite resources, we are open to doing things differently, and through the use of technology and collaboration we aim to build on the strengths of our existing GP practices to further enhance local service provision.

Our Strategic Goals

  • Improve the patient experience of care (quality, access and satisfaction)
  • Improve the health of the population (health outcomes)
  • Improve productivity by reducing the per capita cost of healthcare (doing more for less); and
  • Improve the staff experience ensuring everyone is valued (achieving an enjoyable work life balance).

Only by achieving a balanced approach to all four of these strategic goals will we have a sustainable and high performing service model and one which is fit for purpose.

Our Collaborations

The challenging environment we work in presents an impetus for practices to take forward new ways of working, to help alleviate pressures and to provide opportunities for practices to collaborate at scale on a range of service issues. The PCN is in the early stages of development, but already we are actively seeking to work in new ways. 

This was never more evident than when considering our response to the recent pandemic. The four GP practices in North Uttlesford, collaborated to establish the Lord Butler Leisure Centre vaccination service to optimise capacity and efficiency, in order to save lives.  Building on this success we anticipate the PCN to develop further initiatives to improve and extend local care delivery.

The following services are being deployed across North Uttlesford; Coronavirus (Covid-19) Vaccination Service: Working in partnership with the Lord Butler Leisure Centre, CVS Uttlesford, Volunteer Uttlesford and the District Council, the PCN established a Covid -19 vaccination service to respond to the pandemic. Recognised nationally as one of the most efficient vaccination sites, we have now administered over 100,000 vaccinations to local residents.

Meet our Team

Leadership Team

Our Primary Care Network (PCN) leadership team brings together experienced clinicians and managers to improve local healthcare. They provide strategic direction, coordinate services across practices, and drive innovation in patient care. By working collaboratively, the team ensures that resources are used effectively, new roles are integrated smoothly, and patients benefit from accessible, high‑quality support tailored to community needs.

Care Coordinator

A Care Coordinator is a trained healthcare professional who helps to manage the patient’s condition and oversees the patient’s treatment plan. They monitor and coordinate treatment plans with patients in order to educate them about their illness and connect them with various medical professionals as needed.

Clinical Pharmacist

Clinical pharmacists are increasingly working as part of general practice teams. They are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach.

Dietitian

Dietitians are uniquely qualified to translate scientific information about nutrition and food into practical dietary advice. As well as providing impartial advice about nutrition and health, dietitians also advise about food related problems and treat disease and ill health to educate them about their illness and connect them with various medical professionals as needed.

Digital Transformation Lead

Oversees digital innovation and service improvement across the PCN, supporting the implementation of new technologies and care models.

Health & Wellbeing Coach

A health and wellbeing coach can support you to improve how you manage your physical and mental health. They work with you through a series of sessions to help achieve your personalised health and care plan goals, offering support and advice about diet and lifestyle.

Pharmacy Technician

Supports Clinical Pharmacists in managing medicines safely and effectively. They assist with medication reviews, repeat prescriptions, audits, and patient queries, helping to improve prescribing processes and reduce workload across the practice team.

Physician Associate

Physician associates support doctors in the diagnosis and management of patients.

Physiotherapist

Physiotherapists help people affected by injury, illness or disability through movement and exercise, manual therapy, education and advice. They maintain health for people of all ages, helping patients to manage pain and prevent disease.

Podiatrist

Podiatrists are medical specialists who have been trained to assess, diagnose, and treat conditions related to the feet, ankle, lower limbs, and their associated structures. Injury, overuse, or conditions causing inflammation involving any of the bones, ligaments, or tendons in the foot, can cause foot pain.

Social Prescriber

Social prescribing is an all-age, whole population approach that works particularly well for people who:

  • Have one or more long term conditions
  • Who need support with low level mental health issues
  • Who are lonely or isolated
  • Who have complex social needs which affect their wellbeing.
